Office Visits
When making an appointment, that time is allocated specifically to you. If you cannot keep your appointment or need to reschedule, please call 732-493-3774 at least twenty four (24) hours in advance on the prior business day. Failure do so may result in a cancellation fee equal to that of the missed appointment.
Payment Information
Payment is expected at the time of visit unless other arrangements are made in advance. At this time, only checks and cash are accepted as forms of payment (no credit cards).
Insurance Information
Dr. Lang does not participate in any insurance networks. If your insurance has out-of-network benefits, as a courtesy, Dr. Lang’s office will submit claims to your insurance carrier. Any amounts not reimbursed by insurance are the patient’s responsibility.
Privacy Practices
Our Notice of Privacy Practices describes the limitations on how medical information about you may be used and disclosed in compliance with HIPAA privacy regulations. A copy is available upon request.
